If the male reports persistent accounts of not being able to climax with vaginal intercourse the diagnosis of delayed ejaculation is made. It must be differentiated from erectile dysfunction in that an adequate erection is present and sustained.

    The medical history should include an extensive sexual history such as when did the symptoms start, in what type of situations do they occur and how does this affect the man’s sex life. An extensive neurological history is also necessary to exclude physical disorders as the cause of delayed ejaculation.

    Further history can determine if a medical or psychological cause of the delayed ejaculation is likely and can guide further medical testing. It is important to determine whether the man is able to ejaculate during masturbation. If he is able to ejaculate during masturbation or if the delayed ejaculation occurs just during certain situations or with a certain partner, the cause is most likely psychological in origin.

    The medication which the patient is on is also very important to note during the history.
